Care Monitoring and Communication Solution
Vicert developed a monitoring app with a structured data entry. Our client needed to optimize their organization's communication, and we offered a solution. Our software reduced the times our client’s staff used on calls and faxes by 85%. We also improved data integrity.

Challenge:
Our client needed an efficient and user-friendly way of communicating information from an assisted-care facility to a medical practice.
Vicert had to develop a communication solution to answer the client’s challenge. Facility personnel spent a lot of time on calls and sending or receiving faxes. Vicert developed software to help the client reduce the time spent on these activities.
The clinical information in question was related to patients’ change of condition. All the information was gathered and transmitted to our client’s personnel so they could inform the correct clinician. Our client developed a protocol that dictated how to properly communicate all the relevant information about the patient’s condition change.
Vicert developed a monitoring app to simplify and optimize the client’s protocols.
Solution:
Our solution had three parts:
User-facing mobile application
Data-processing backend
Admin web portal
The mobile app serves as a data-gathering tool. The assisted-care facility personnel enters all the necessary information regarding a change in condition, which is relayed to our client.
The user interface for the data collecting was implemented in a user-friendly manner, leading the user step-by-step through the appropriate protocol. We’ve implemented our patient monitoring software to build on the values entered in the previous steps of the protocol.
The mobile app was implemented using React Native, available for iOS and Android, and published to the appropriate stores (Apple App Store and Google Play). The app’s security included a token-based authentication schema.
Following the submission of data, our client’s personnel were able to review it. Using the web portal, they could quickly analyze the data and then notify the appropriate clinician, depending on the situation's urgency.
We’ve developed our monitoring app with an admin portal using React, with all the necessary security, including the proper authentication and role-based authorization.
The app backend enabled:
Receiving the data from the mobile app
Generating a PDF form of that data
SMS sending capabilities to notify clinicians
Integration with the EHR (eClinicalWorks) to electronically submit the protocol data and associate it with the appropriate patients
Our communication software uses the API architecture designed using next-generation lightweight frameworks based on Java. The backend and admin portals were deployed on AWS.
Benefits:
We’ve saved a substantial amount of our client’s staff time by improving their organization’s business workflow.
We’ve reduced the number of calls and faxes our client received from the assisted care facility by 85%.
We’ve improved data quality through a more organized and structured data entry via the new mobile app.
Our communication solution enhanced the overall care management system via the increased speed of information exchange.
